Please read this Informed Consent for Telehealth Services (“Consent”) carefully before using Harbor Healthcare’s telehealth platform. By using our Service and participating in a telehealth consultation, you acknowledge that you have read, understood, and agree to the terms outlined below.

What is Telehealth?

Telehealth is the delivery of healthcare services using electronic communications, including video conferencing, secure messaging, and other digital technologies. Through our platform, you can consult with licensed healthcare providers remotely without the need for an in-person office visit.

Telehealth consultations through Harbor Healthcare may include:

  • Review of your medical history and health questionnaire
  • Synchronous video or audio consultations with licensed physicians
  • Asynchronous review of your health information by a licensed provider
  • Prescribing of medications when clinically appropriate
  • Ongoing monitoring, follow-up, and medication adjustments

Benefits of Telehealth

Risks and Limitations of Telehealth

As with any medical service, telehealth has potential risks and limitations, including:

  • Technology limitations: delays, interruptions, or failures in electronic communication may occur
  • Diagnostic limitations: the inability to perform a physical examination may limit the provider’s ability to diagnose certain conditions
  • Security risks: despite encryption and security measures, electronic communications carry some risk of unauthorized access
  • Not a substitute for emergency care: telehealth is not appropriate for medical emergencies — call 911 or go to the nearest emergency room if you are experiencing a medical emergency
  • Prescribing limitations: certain medications cannot be prescribed via telehealth under state or federal law

Your Responsibilities

By participating in telehealth services through Harbor Healthcare, you agree to:

  • Provide accurate and complete health information, including your medical history, current medications, allergies, and any relevant health conditions
  • Inform your provider of any changes in your health status
  • Follow your provider’s instructions regarding prescribed medications, including dosage, administration, and potential side effects
  • Seek emergency medical care when necessary — telehealth is not a substitute for emergency services
  • Ensure you are in a private, safe location during consultations
  • Notify Harbor immediately if you experience adverse reactions to any prescribed medication

Provider Qualifications

All healthcare providers on the Harbor Healthcare platform are:

  • Licensed physicians (MD or DO) in good standing
  • Licensed to practice in the state where you are located at the time of consultation
  • Board-certified or board-eligible in their specialty
  • Credentialed through Harbor Healthcare’s provider verification process

Prescribing Practices

Medications prescribed through Harbor Healthcare are based on the treating physician’s independent clinical judgment. Not all patients will qualify for medication. Your provider may:

  • Prescribe FDA-approved GLP-1 receptor agonists or other medications for weight management
  • Adjust dosages based on your response to treatment
  • Decline to prescribe medication if it is not clinically appropriate for you
  • Recommend alternative treatments or refer you to an in-person provider

Pharmacy and Medication Delivery

Prescriptions issued through our platform are fulfilled by licensed, regulated pharmacies. Medications are shipped directly to you in discreet packaging. You understand that:

  • Shipping times may vary (typically 2-3 business days)
  • Temperature-sensitive medications are shipped with appropriate packaging
  • You are responsible for proper storage of medications as directed

Recording

Telehealth consultations may be recorded for quality assurance, training, and compliance purposes. By consenting to telehealth services, you acknowledge and agree to this recording. All recordings are stored securely and handled in accordance with HIPAA requirements.

Right to Withdraw Consent
